ART Integration Puts HIV Treatment on Ventilator

KOCHI: If not handled with care, the state government's move to integrate the highly successful 10-year-old Antiretroviral Therapy (ART), the programme for treating HIV patients, with the general health system might prove disastrous.
Once the integration happens, there are possibilities that many intricate confidential information of the patients will have to be disclosed.
Incidentally, the confidentiality factor was the key to the success rate of ART in the state, which could proudly push aside other states in its implementation.
Unlike other diseases, the HIV prevention programme is not included under the Health Department of the state, but is managed by the Kerala State Aids Control Society (KSACS) which is directly under the Department of AIDS control, National Aids Control Organisation (NACO).
The HIV is the only disease which has a prevention programme outside the General Health system.
Study shows that there are about 25,000 HIV patients in the state of which 18,000 have registered with eight centres in the state and 9,000 are undergoing treatment.
"The patients require close monitoring. Even the slightest interruption in medication can make the whole effort go awry. Sadly, even after giving meticulous care, there are drop outs. The confidentiality cover might dissolve if the programme is not properly integrated with the general health system," said a KSACS official on condition of anonymity.
There are strong apprehensions whether the integration process would in any way meddle with the welfare schemes of HIV patients. "Implementing it through various agencies of the government will force them to disclose the patients' identity. But NACO, through an efficient screening system, gave away the benefits to the patients adhering to confidentiality guidelines," the official said.After integration, the funding will be direct from the Centre to the state. The state will inturn release it to treatment centres via treasury, unlike the old system where NACO directly releases funds to treatment centres.
The lag being a major barrier in the effective implementation, the official pointed out that KSACS is yet to get the fund for the fourth phase. "Seventy five percent of the aid should be utilised before six months. If the money is sanctioned towards the end of this year, the projects will have to be implemented in a haste which can affect the quality of the programme."
Above all, the frequently changing guidelines is also giving headaches. "The existing infrastructure is not suffice to meet these demands along with the changing guidelines," said Dr Ajith Kumar, member, technical resource group of NACO.
KSACS joint director (Care and Support) Dr T V Velayudhan, assured that the integration would in no way affect the HIV patients. There will not be any change in the medicines and treatment, he said.

NACO Launches Lifelong ART

Following World Health Organisations’ recommendations, the National AIDS Control Organisation (Naco) has launched lifelong Anti-retroviral Therapy (ART) to prevent the transmission of HIV from an infected mother to her unborn child.
Under the programme, triple drugs will be administered to the HIV-infected woman from the early stages of pregnancy and will have to be taken lifelong.
According to experts, Anti-retroviral Therapy will bring down the possibility of transmission of HIV from mother to child to 1 per cent. Besides, the mother will also be able to breastfeed the child.
The triple drug consists of tenofovir, lamivudine and efavirenz.
Prior to this programme, only a single drug (ARV prophylaxis with single dose of nevirapine) used to be given to the HIV-infected mother at the time of delivery.
This dosage would reduce the risk of transmission by 35 per cent to 15 per cent. Moreover, breastfeeding was discouraged.
Dr Velayudhan, joint director (care and support), Kerala State AIDS Control Society, said apart from giving prominence to Anti-retroviral Therapy  in the programme launched by the Naco, it also give focus to certain other aspects too.
“Apart from giving lifelong Anti-retroviral Therapy, it also intends to prevent HIV in women, prevention of unintended pregnancies in infected women, prevention of HIV transmission from infected mother to infants and provision of care, treatment and support to mothers living with HIV, their children and families,” he said.
This programme intends to intervene in the community to bring down the incidence of HIV, said Dr Ajith, Additional Professor, Thrissur Medical College.
“A comprehensive counselling is also needed to prevent them from being brandished as HIV patients,” he said.
While most of the developed nations have already adopted this approach, India had been avoiding it on concerns about the feasibility of Anti-retroviral Therapy.
Even the World Health Organisation had highlighted the need to shift to this programme to the Central Government.
“Compared to the triple-drug system, the single dose drug is comparatively cheap but not too effective. Besides, if the infection progresses to a later stage, the single dose drug proves useless against it. In many parts of the country, antenatal care is very poor,’ he said.
“To be able to get the drug in the initial stages, an HIV-infected woman has to register in the programme. Hence the programme was found to be not feasible. But now the Naco has decided to follow the triple-drug system,” he said.

Govt to Issue BPL Cards to All HIV +ve Patients

The government has decided to issue BPL cards within 60 days to all HIV infected patients.
In a meeting attended by Chief Minister Oommen Chandy, Health Minister V S Sivakumar and Social Justice Minister M K Muneer at the Secretariat, it was announced that the government had formulated several policies to protect the 8,227 HIV positive patients in the state.
For patients undergoing Anti-retroviral Treatment (ART), the monthly aid has been increased from Rs 400 to Rs 1,000.
Kerala State AIDS Control Society will be entrusted with a corpus fund of Rs 3 crore towards this purpose, according to a release.
"Children of HIV patients will be provided free education and scholarships. There will be no age limit for pensions given to widows of AIDS patients," it said.
All tests will be free for HIV infected patients in the labs run by hospital management committees in various government hospitals.
In case a hospital doesn't have the provision for a particular test, the government will take measures to reimburse the money spent on tests.
To ensure the financial security of HIV patients, the Social Justice Department will introduce self-employment schemes. The government is also envisaging a housing project for them.
Medicines that are not available at the eight ART centres in the state will be made available through Kerala Medical Services Corporation. In the eight ART centres in the state, counsellors to implement Social Justice Schemes through single window clearance would be appointed. Schemes to provide nutritious food would be re-introduced in all ART centres, according to the release.
Special training will be given to those willing to provide nursing care to HIV patients.
AIDS awareness will be part of the school curriculum.
The Social Justice Department will protect the rights of HIV patients, through a health centre to be established in Kochi. The government has also decided that once in three months, the Chief Minister, Health Minister and Social Justice Minister would meet  to analyse the issues which affect HIV patients.

PALAKKAD:  Palakkad will be declared the first total HIV/ Aids literate district in the country on Monday. T N Kandamuthan, district panchayat president, said that in the last nine months a scheme titled 'Jyothirgamaya' was implemented under the district panchayat and a voluntary organisation KESS-HAPPI, towards making the district HIV/AIDS literate.
He said that the programme was implemented with the co-operation of 91 grama panchayats and 4 municipalities. The aim of Jyothirgamaya was to educate people about AIDS in reducing stigma and discrimination, protecting and bringing the affected to the mainstream of life and rehabilitating them.
Under Jyothirgamaya, Asha, anganwadi and Kudumbasree workers distributed notices in every household, conducted awareness classes, quiz competitions and rallies at the school and college level. Boards and wall writings were put up at all vantage points. A steering committee was constituted at the district panchayat, grama panchayat and block panchayat level to take forward the activities of Jyothirgamaya.
He said that these activities were undertaken in the district from January 2015 to April 2015.
